PREFACE TO THE PREVIOUS EDITION 

So much has changed… 

This 9th edition of Moss and Adams cardiology text was developed approximately a year earlier than planned to bring the readership up to date on the many new findings in our specialty. New chapters include Development of Myocardial Structure and Function (this amazing chapter makes having the book worthwhile on its own), Global Challenge of Heart Disease, Impact of Congenital Heart Disease on Other Organ Systems, Noncompaction Cardiomyopathy, Beyond the Device and Transition of Care for the Young Adult Who Has Congenital Heart Disease. Many of these new chapters resulted from suggestions offered by the readership. Additionally, new authors have offered fresh and exciting perspectives for several other chapters. 

The enhanced digital version of the text allows us to present many of the graphics in a video format, allowing visualization of echoes, angiograms, aortic arch anomalies, and even open heart operations. Additionally, we have included phonocardiograms with accompanying audio playback of various heart sounds and murmurs. The audio-phonocardiograms will be found in Chapter 9 and can be applied to lesions throughout the text. These were recorded by Dr. W. Buck Kyle and myself (HDA) on various patients using a Littmann 3200 digital stethoscope. Murmurs were best demonstrated using the stethoscope’s extended range mode. The recordings were uploaded to Littmann StethAssist, then exported in mp3 format to Audacity (web.audacityteam.org) for editing. The phonocardiogram created by StethAssist was imported into Camtasia (www.techsmith.com) to allow for editing. With help from Freddie Patane, Art Director, Media of Wolters-Kluwer, the audio and video were combined and optimized then uploaded to Vimeo (www.vimeo.com) for online access. High fidelity headphones are highly recommended as important qualities of the sound will be lost or distorted with speakers or standard headphones. 

We have new Associate Editors, Drs. Dan Penny and Frank Cetta. I also thank Drs. Shaddy and Feltes for their ongoing participation and advice. Dr. David Driscoll’s previous editorship was invaluable and we publicly thank him for his hard (and timely) work. On a sad note, we have lost two close friends who were previous editors of this text, Dr. Tom Riemenschneider and Dr. George Emmanouilides. They are sorely missed. Dr. Forrest Adams, first editor of this text, continues to be active and to offer insights into cardiac physiology. He is an inspiration to us all. This will be my last opportunity to serve as lead editor of this text. It has been an educational and rewarding experience. Thanks to all of you.
